This is the voting form for the second annual EPW year end awards. We will be asking, YOU, the EPW fans to vote for on various categories. The awards will be presented at Re-Awakening on November 6th 2004. All entries submitted will be eligible to win  a $100 voucher for EPW merchandise so please, have your say in this year's end awards.

This award can go to any wrestler who is not listed in the Rookie of the Year balloting

Quite obviously this award is to see which event was the most entertaining and action packed. The events this year have been/or will take place before balloting closes:

Match of the Year
This award is for the best EPW match to take place since January 1st 2004
